The shape of the top of pad is not conical but spherical, machined to a certain radius.
I put your dimensions into a drawing (PDF, all to scale) and found that said radius is more like 5.0"

But since the OP's question was about "same lug spacing"..., my answer was two - which of course means NO, not the same for all (20") wheels.
Military or not, for truck wheels 20" and up, there are only two different patterns (= pitch circle diameter) in use, out there: 10 on 335mm and 10 on 285.75mm (= 11-1/4").
First one is the most popular, by far. I'd say it's 99% of all big size trucks in the world.

Some of those are loose and fall out when you remove the bracket; others are a tight fit, and then some are like solid, welded in place... But that's only from rust built up.
So don't be shy and just use a bigger punch and heavier hammer, with the bracket firmly supported. PB-blaster; Rust-Off...

Correct. Red line for rpms is there for a reason; to keep things on the safe side. If you wanted to go higher for extended periods, rebuild the engine and do something the military doesn't care about, that much: have every rotating part perfectly balanced!

Sure, but that would be more like momentary contacts only, at low speed (<15mph). And it is clear that with extreme low TP the increased flexing work alone will create a lot of heat in the tires. Not exactly what I was referring to as "normal use".

1 tube per tire should be sufficient, if you manage to apply an even, 4-in wide bead to the crown.
It should be noted that the runflat inserts (all versions) are always much smaller in diameter than the inner tire. So, for the 395s and 14.00s it takes about 5" for the tire to actually touch the RF, so that would only occur after much deflation, e.g. during emergency use; or a mayor air leak...
TM -34 on p. 3-17 reads:
