Cab rust fixable or look for a new cab? m35a2

Is that the tip of the iceberg, meaning how are the sills under the doors? If those and the rear cab panel area are all rusted out then yes, swap cabs.

if memory from a few weeks ago crawling around serves me correct that seems to be the only place around the cab with a major problem even the bottom didnt look too bad

Well in that case that is fixable. Remove the dashboard and remove all rust, get a piece of angle iron to back it from underneath, attach it with rivets, epoxy, screws, welding. Prime with cold galvanizing paint, then topcoat.
