Lack of maintenance? Looks like the throwout bearing may have been seizing. They do have grease fittings but are very often overlooked. Perhaps the assigned driver used the clutch pedal as a foot rest.

Sounds like the pressure plate is coming apart. Happens somewhat frequently. Symptoms describe it exactly. Need to get someone with a towbar out there.
