Total brake failure during a panic stop
Judging by the pictures, I'm going to hazard a guess that it was a bad weld on the shoe. When you hit the brakes hard, it was enough to push the flange out and that is what broke the tab on the wheel cylinder, since that seems to be a shiny, fresh break. I agree that it looks like you had a...
