    I am on active duty in Az. I am living in an apartment paid for by the government. The phone is not in my name, billing address is differnet then apt address. I sent a copy of state ID with AZ address on it. And a copy of Military Id, Passport and UT drivers Lic. I have waited 41 days for EUC...
