250 cam thrust plate and timing cover busted

What would cause the cam to push toward hard enough to bust the cover but the motor still runs ?
Hey any ideas on this?
Wrong thrust washer spacing, according to the TM, there is Two different cam thrust designs

From tm-9-2320-160-34-1, page 3-208
"Measure shim(s) (12) as a pack with micrometer. Add or subtract shim(s) (12) from pack to provide
a clearance of 0.001-0.005 in. (0.0254-0.127 mm) for engines with camshaft thrust plate or​
0.008-0.013 in. (0.2032-0.3302 mm) for engines with camshaft outboard bearing support".
