Model: CUCV, K-5, or Blazer

Body Style: SUV or Blazer

Weight: 5200

GVWR: 6400

Comments: N/A or blank

I used the following:

Year -- 1984
Make -- Chevrolet
Model -- M1009
Body -- Blazer
Weight-- 5,200
GVWR --6,400
Comments -- NONE

Went through GL and PENN DOT without any problems in getting a PA title for the vehicle.
