As requested, here's an example of the export letter.
Please submit the letter with a completed EUC form if you're a first time buyer, or if you're a returning buyer purchasing Demil BQ material and you have not previously submitted this letter.
Once the letter has been received, a copy will be kept on file for subsequent EUC submissions.
Please include your customer#, name [or business name] and address at the top of the letter. Then sign and date at the bottom.
A scanned, digital submission, such as a jpg, is acceptable.
Faxes can be sent to 480-596-4480. Scanned images can be sent to eucinfo@govliquidation.com
