Rear Window Regulator and clutch slippage

A few months ago I took out and overhauled my rear window regulator and handle.

At the time I took out the regulator and greased and oiled it up and replaced the tailgate handle with a new one from LMC. So the clutch and tailgate handle are new.

A few weeks ago the tailgate handle wouldn't catch and turn the regulator crank. It would just spin and not catch.

Today I took the regulator and handle crank back out to inspect. The clutch seems to be in good condition and little worn, but nothing bad. The regulator also seemed to be in good condition. What I did notice on the regulator part that connects to the clutch on the handle were bent inward. Would this cause the clutch to slip and not catch? This doesn't look normal to me.
