Measure the outside dimensions and then mark with a Sharpie. Place duct tape and some 90 lb. felt along the "cut" line. Use a high power sandblaster to "cut'" through the glass at the edge of the tape and felt...

For the glass itself just go to an Auto Glass supplier and get a piece of safety auto glass cut to size. It is just a piece of flat glass that is so easy to accommodate.
