The plate is a simple affair and can be easily made with some aluminum or steel scarps and drill the holes through. The OE is roughly 5 x 3 with a cut out.
The riser pictured in the bed is typically on slant backs.
There is also rear fender top mounted antenna towers if you want it higher. I have seen the rear bed cover modified with 4 bolt holes and the center cut out to use the antenna tower with the cover on. I have driver's side antenna towers...
That is a very interesting way to use the mount. It typically has the plate on the side fender and the mount to the rear and the offset to the right or left on top of the mount.
Part of this trouble shooting sequence would be to bleed the brakes. Just unplugging the switch will not remove air or indicate the master cyl is bypassing. Nothing special to this being a hmmwv in standard brake troubleshooting just like any vehichle. We are not working on the hydroboost side...
Easy to find. You can use the square or half moon lock pins. Home depot, Lowes, tractor Supply. OE also uses a metal wire rope crimped on both ends on to the pin assembly so it does not get misplaced. Also easy to make with stuff from Home Depot or lowes.

I would replace also while open.
Clearly the filter was not changed and everything else done would be suspect and needs to be verified.
I would give him the bill for the filter and separator and demand a refund plus labor.

typically the covers I have seen are black rubber covers that slip over. The bottom can be cut to make it easy to slide on after everything is on.
This vendor on ebay has it and is reasonable plus not stupid crazy shipping. Used him many times :)...
