That would be the factory setup on a '91 Crew Cab. Of course, only about 1,500 of those were made.
If you can't find the rods, making your own should be easy enough.

FWIW this information is from another site;

"The 6 bolt one is for 4wd transmissions and the 4 bolt is for 2wd. The 4 bolt lacks the bosses for the support rods the 6 bolt cover has."

The GM part number for the 6 bolt cover is 15671930

If you can't locate one, consider making one - or having one made - from a common 4-bolt version.
It's only a matter of some welding and minor machining.

Highy unlikely you will crack it unless you are doing dukes of hazard jumps. gmwholesale partsoneline.com has them. Just look for a Late 90's k2500 with a 6.5 diesel. You will need the plates behind the motor mounts along with the two rods. There is a parts diagram so you can see what you need.

Not that I've looked into the correct parts, or interchangeability, but it seems to me that it shouldn't matter what engine is in front of the transmission.
A 4L80E cover for a 4WD application would likely fit regardless of what engine is in front of the trans, or what transfer case is behind it.

And I'm getting the impression that you will not entertain the idea of making any of these parts, not even simple rods or plates, right?

Try GM PN 15529883 I Just got these through wholesale gm parts online. They shipped from Bob Hook Chevrolet. I was wrong about the flywheel cover. I have both and I compared the two. The bolt holes are in a different location. I just bought these this spring. Even though a PN might show to be discontinued they can sometimes find it still at a dealer's shelf. I will look for the invoice and get the part numbers for the plates behind the motor mounts.

This is for the drivers side plate that goes between the motor mount and the block.
