Will post on both threads..
DOT and five different truck and auto shops observed the following:
<> Spacer blocks to high and hollow, not solid. If "A" block was to be used for a spacer it should have been solid and a bolt from the spring set attached through it.
<> Main leaf spring hangers (2 on each side) on secured with two 1/2 grade 8 bolts, only spot welds on the bottom of the frame
<> Leaf springs only rated for a ton or maybe less
<> No support bracket installed securing u-bolts to the top of the springs
<> Torsion spring improperly installed and only welded to the block sets
<> When the tandem (boggie) axle was removed, the bolts used to secure the axle also served to secure the inner rear frame of the truck. Now there are just holes in the frame. Those bolts were never replaced.

And the list goes on and on and on and on and on!

A lesson on how not to bob a five ton!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!

Providing detailed information on the issues from the DOT , would be good information to post to help other avoid the same problems . The link to the build thread can be a great what not to do when bobbing link , but only if you post what's wrong.

I'm going to merge your threads. You are posting duplicate info in them and referencing each of them.

You should have simply continued your original thread.

I found the Failsafe Monitor Module in the TM, but it doesn't describe it's location. Is it under the dash or hood? Hopefully it's just a bad ground or connection.

It is under the hood. The ground is through the mounting spacer. Take them out and make sure it is clean of paint and scale all the way through to the firewall.
The failsafe module is NOT under the hood, it is in the left front kick panel in the cab, right above the fresh air vent held on by 2 7/16 bolts,1 ground wire and a large screw on connector with wires coming in the top of it.

Thanks twinturbo90 for the correction, sounds like wheelSpinner might have been referring to the PCB. Thanks all!
