Need help identifying this turbo, and what motor it most likely came off of.

Day late and dollar short on this reply. That is certainly an LDS 427 turbo. Dead giveaway is the bulky inlet compressor body. I've heard that those are the cat's Meow of turbo's on a Deuce.

I thought the 427 turbos had a flat spot on them right about the top of the compressor side.

Schwitzer 4D454C, like the data plate says. Used on late model LDS 427-2 engines.
