PTO data Plate for M37

Say can someone send me a picture of the PTO data plate for the M37? I"m looking for one. Thank you

This is the plate on the left side of the dash, the part numbers are on the envelope.

They also came with a black background instead of the red.

The winch operation/caution plate. OK, I have that. The PTO itself has no plate.

Thanks for the picture, any ideals were I can find one? I did call around to the places that might sell them but I always keep getting the "Good Luck In Finding One" even through they list them for sale, LOL
