That is a low-tech reversing alarm, when the truck goes in reverse there should be a weight inside there that falls repeatedly and rings the bell. I love those old tech ringers.
Can you hear the contact? Especially if the injectors are out you should be able to ear up to it at each cyl and hear it. If you can't have you double checked any external interference? Flywheel area or such?
