Check that your Blackout switch is set to 'Service'. Then check that you actually have 12 volts to the junction block on the drivers side firewall above the glow plug solenoid.

This was my issue. Everything checked out but had random flashers, stopped flashing with brake depressed etc. Replaced BOTH the flasher relay and hazard relay. Problem went away.
