If you convert it to 12 volts, I would just run one alternator. Unless you have a big electrical load and need the extra amp capacity, there's no point in wearing out two alternators at the same time.

Here is an easy way to remember thread direction for removing the lug nuts. With the lug wrench on the nut and the handle pointing straight up, pull towards the back of the truck to loosen the lug nuts. This is true for either side.

If the alternator is working at all, I would suggest you rebuild the stock alternator. Isolated ground alternators are hard to come by.
