I would check out the timing chain to see if it has broken or jumped some teeth. If you have good spark and nothing happens with ether, there has got to be something wrong with the timing chain or valve train.
I think you definitely need to pull the transmission. When you do, I would go ahead and replace the clutch and pressureplate even if they look good since the parts are cheap and you will already have expended the labor. I'm suspecting broken fingers on the pressure plate.

You can normally hear the fan engage over the sound of the engine. I never have mine engage while driving, but if the truck sits and idles for a few minutes after warm, the fan will engage.
