Ok good. My next efforts would be to make 100% sure there are no hairline cracks in any fuel hoses that could admit air. As fuel demand is increasing the air entrainment get so bad the engine falls on it’s azz.
The Michelin 14.00’s are notorious for blowing out. If I were in your situation, especially if I had a heavy expandovan, I would switch over to Goodyear MVT 395s already mounted on FMTV wheels. Feltz Tire is a good source for these. Two pallets of 3 tires each will run you around $350...

The TCM most definitely takes into consideration the throttle position via the TP (throttle position) sensor mounted to the injection pump. It sends a signal of 0.5 volts at closed throttle to 4.5 at wide open throttle. I would suggest you perform diagnostics. 2013 was a long time ago.
The return line tees and supplies both pump’s reservoirs. Sounds like all the fluid is being forced back into the hand pump reservoir. I would study the hydraulic schematic looking for possible culprits like stuck closed check valves.
