This box should be available from heavy truck used parts suppliers nationwide. The plate would have to be manufactured by whomever required it as they were part of the trucks frame assly when new. It doesn't look like it would take too much skill to fab one up. As well...

You should only be engaging the front axle when it is required for better traction on loose or slippery surfaces to assist in moving the vehicle.
The MLVW does not have any locking axles on it in the locking differential sense, if that was what you are...
