I used a Dodge 360 with a NV4500 transmission, 5 speed with overdrive. I kept the original brakes and added a booster. Brakes properly adjusted and with the booster will put you through the windshield if stomped on!

No, I kept the 5.83s, however the NV4500 transmission has 5th speed overdrive and the tires are 38" tall. Cruise easily at 55 to 60, going faster is not comfortable in this vehicle.
MPGs were never a concern, so I have no idea of my mileage.
The NP200 transfer case is not as weak as you may think, provided the lubricant level is maintained. MyPowerwagon and m37 used the stock transfer case. Power Wagon had a 440 engine, M37 had a 360 Dodge engine.

How are you checking TDC for number 1? You can't use the spark plug hole as it is not over the piston. Number six has a plug in the head that is over the piston that is used to find TDC. As stated above number 6 and number 1 are in the same plane as determined by the crankshaft.

It appears to me that the pivot bracket is missing or broken off. The welded bracket on your floor pan tunnel was made to replace the missing pivot.
