That is definitely a hydraulic pump noise, could be the trans but most likely the steering pump, some are just noisier than others, if it were the Cadillac valve the noise would drastically change as the fan turned on and off.

Your thinking is correct, that "death rev" is a pretty sure sign of lack of fuel. You need to be careful pressurizing the tank, too much and you can blow off vent lines. It could be the lift pump. When was the last time the fuel filter was changed?
