Any of the big truck supply shops can fix you up with this setup. It will probably be a bit more pricey than what was stated above. I got lucky and was able to pick up a Gov. surplus one, but had to replace the rubber seal.

Sorry, you had a good picture. I thought if they weren't familiar with the area, they may know where Olympia is. I have driven exit 82 to 132 most days for years and I couldn't tell you that was 93rd St. To me it is the truck stop exit south of Tumwater.
For Reo54 and Choll, it didn't look like anyone directly answered your question about Exit 99. It is about 4 to 5 miles south of Olympia on I-5. Highway 16 is about exit 132. Exit numbers correspond to the miles from the Oregon border on I-5.
