Try backing off the adjusters, soaking in penetrating oil for a day, and beating on the drum with the outer bearing out. Use a wood block to keep from marring the drum. Beat on it for 30 minutes if you have to, it will break loose.

No harm will come from driving the truck with the PTO engaged for a short while. All the PTO does is engage the end of the mainshaft of the transfer case with a sliding dog clutch.

Well lesson learned, any time you have a "very loud knock", that is a precursor to something even worse, now you will find out what that is. Sounds like something was ingested and now you have a busted piston, scored liner, hammered valves and head.
