Kind of a tighten, spin a few times and back off a quarter turn, type deal, before replacing the cotter pin, or at least that is what I do. By no means a master mechanic, just what I do since burning out bearings on my car when I was young for having them too tight.

I've heard how much effort it is to get run flats into the tire, I can only imagine how fun it is to remove them. I think removing them would be the last option, unless you truly do not want them in the tire.
