If I understand the engineering intent of the heavy wall spacer correctly; it allows for a little flex without accumulated stress cracking the frame (Just like the ones for the bolts on the steering gear box protect the frame).

A high leverage hand tool will set you back around $300 with a single mandrel. You can get any kind of thread-sert you want if you hunt hard enough. I use aluminum, steel or stainless inserts depending on application and even have some for blind holes (including in thick Lexan)

As stated, the full braking ability will be there without air. It will take as much as 10 times the pedal pressure and all bets are off if the brakes have a major leak or they are otherwise inop.

I suspect none of them deal with the initial installation. If a winch was added after manufacturing the truck, the winch kit would likely have come with a cut sheet set of installation instructions just like a heater, hard top, air shift transfer kit etc.
