Many low-mid quality multimeters have trouble reading frequency correctly on generators, even some high end models are known to have issues. This can be caused by skewed waveform, noise, induction from the other phases, etc.

Sound like you may have a problem with the voltage regulator, there should be a test procedure in one of the manuals for manually flashing the fields and seeing if you get a spike of voltage.
