If the flanges (or output shafts) and those on the receiving end aren't close to parallel, it will be hard to keep joints in it. Essentially, it tries to make the ends of the driveshaft turn at different speeds as the axis is shifting...
A simple ID of the safe wheels is the ring is stamped "LTS".
Those are very hard to assemble incorrectly.
$100 worth of tools and you can learn how to do it yourself and never depend on tire shops again.
I've been doing my own for over 55 years now and NEVER had a close call with them.

Fab a 90 degree bracket, with the top flange lining up with two of the bolts on the bottom of your 5-ton front bumper driver side, the other flange drilled to accept the 4 bolts of a regular pintle hook.
Put it together and you'll be SO HAPPY moving trailers around that way.

Maybe get a pair of M34 (M135) wheels, they have the rim set out a bit, and were designed to run as singles all around.

I cut a notch in all my 20-bolt wheels and run radial tubes/flaps on my M54A1.
I could never keep air in them otherwise. My wife made planters out of all the rubber inserts...

Welcome the the finest in 60A alternator design. (not).
Prestolite really got into the gov contracts with that design, and those are very prone to exactly the type of failure you experienced.
