5-ton injectors have one hole. 2.5 ton injectors have two holes. The fuel delivery is different and so in the spray pattern, among a hos tof other things.
The 5-ton engine also has differnt pistons, AND MOST IMPORTANTLY different "oil squirt" piston cooling nozzles to keep the fire deck...

The problem is the 855 CI block (which this most assuredly is) won't fit under the hood of either the M39 or 800-series trucks...too tall. I've seen it done, looks really odd with a channel cut into the hood. The same problem exists when putting a Jake brake on the Cummins 250.

A number of dueces inherit 5-ton multifuel engines, with marginally better performance. The main problems are overheating...
No, it is just the vent system occasionally gives grief. Seldom. Just a thought. Fuel gets into the MC. Brake fluid gets into the engine. I've even seen gas in an M37 distributor 'cause the owner didn't capiche how the plumbing was supposed to go, and the thing burst open.

Maybe not the culprit, but take a look at the master cvlinder vent system. I've seen them go goofy and pump BF into the engine intake system, with a lot of white smoke. Usually caused by a problem in the air-pac.
