I would consider using a commercial crew cab and transplant it instead of trying to mod the exisiting cab.
Maybe an International or the like. Or a Pete or KW with a big studio sleeper.

I have a M275 and have had an M54 Multi-fuel 5 ton and an M-915.
The M-915 has it over the others hands down...there is no subsitute for HP and higher gears.
