I too have thought of the air cut out. I've even purchased the parts. But was planning on installing it when i install the new dash. The previous owner a, Fire Dept butchered the dash installing switches, lights and radios. I even got a new dash. Looks like it's getting moved to the front burner.
One other item that has been over looked. Make sure your oil filler cap on top of the valve cover is in good shape! The neiboring fire dept to us lost thier engine when water filled it during tropical storm allison.

Are these two openings your are talking about rectangular? And on the new intake manafold gaskets. If so these are for EGR ports on "C" series engines. Leave them blocked.

They one i had came with rungs welded to the back and they alinged with rungs on the tailgate. You could easily climb from the ground over the tailgate and into the trailer. No slide out provision though. Mine was built in 67
