I checked the manual, the A2 lower gears (CTIS) have a different part number than the lower gears for the earlier (non CTIS) trucks. I don't know if this is only because the CTIS ones have a higher load rating than the older ones, but I also couldn't find anywhere that said they are...

Ground is there on appliances as a safety in case the wiring somehow has an issue and puts electricity to the case. Gives the electricity a very easy path to use instead of going through the human body. Not having a ground won't affect operation of an appliance, only how badly you get...
if it's 5kw element, it isn't going to be 120V.
Put one hot lead to each terminal (L2 hooked to one side, L3 hooked to the other), that gives you 240V.
No stipulations anywhere I can see on the website about on or off road use only.
As for the second part, the contact info for them is on the website. Worst they can really do is tell you "no"
to eliminate the belts as cause of the noise, you can use a spray bottle with some soapy water in it. If you spray and the noise goes away, you know it's the belt. If no change at all, then it's not the belts. quick and easy way to eliminate them as a possible source.
Just went through this...
1) If the top is new then there should be no reason to use anything to waterproof it. If it leaks then something is wrong.
2) soapy water on the piece that goes into the front channel can help things slide easier. Also easier to use 2 people to install that portion, but not impossible with one.
Only issue I see with this is you'd have to do it as a 3 phase distribution/breaker panel. Not necessarily a bad thing, but you still have to double check that the selector switch is positioned properly before starting unit and connecting load. That's pretty much the only reason I didn't...
You could also possibly install it lower and at an angle, but to me it looks like if you go with the angled flat upper portion flat against the back wall you lose a lot of floor space on top of the tunnel, and possibly get some vision blocked. Only upside I see is still being able to look out...
in "H" power goes to all wheels, but if for some reason one wheel loses traction, then all the power will try to go through that one wheel, and the truck won't move.
The "HL" and "L" modes lock the transfer case so that power will always go half to front half to rear. If one wheel loses...

1) 208 3 phase will just put 120 to each leg same as 120/240, but will only see 208V instead of 220/240. Most equipment is somewhat forgiving, you're still within 10% of 230 (240V can actually be anywhere between 220-240). Shouldn't cause any issues but your 240 equipment might try drawing a...
yup, not an exact science. Just anything stiff and flat that won't break if it gets a little weight on it. if using wood I'd recommend something like 1/4" plywood of a decent quality, then paint it to keep it from rotting. Bonus to adding something like this is you can attach things to it...
on my M1123 (2009 rebuild) it's just behind the passenger side wheel well on the outside of the frame. There is a bracket from the Marine Armor Kit that was left on my truck directly in front of it.
I'm running in 2 man setup, the sheet I had was 5 feet long, then just cut to the width needed to fit from windshield to B pillar, then about 1 or 2 inches less. It's held in place by the velcro straps going through it on the support bar and the weight/friction from the top itself.
plywood sheet or hard plastic between the tarp and the bows. I'm using a sheet of 1/8" fiber reinforced plastic for mine with holes cut out wherever the straps hold it to the bows.
